2. Download the enrolment form below. Fill & Sign using Adobe Reader online. Save with your name.
3. Download and sign the Terms & Conditions and Liability Waiver. Save with your name.
4. Send both the liability waiver and completed enrolment form back to: firstname.lastname@example.org with “ ENROL IN ADVANCED COURSE, dates & your name” as the email subject. Eg: "ENROL IN DEC ADVANCED COURSE SALLY JONES"
** If you're not tech savvy and find it hard to use Adobe reader to Fill & Sign the forms online then you can print them out, fill out in black or blue pen, take a photo with your phone & email it to the above email address. Please DON'T send enrolment details in a Facebook or Instagram message.
5. The remaining $1500 must be paid via bank transfer and paid in full at least 2 weeks before course start date. If you wish to pay in 5 installments of $300 please call me for approval (there must be at least 5 weeks, fortnights or months left before the course start date to pay the remaining course fee depending on whether you wish to have a 5 week, 10 week or 5 month installment plan). **All bank details are on the enrollment form.
